In September 2007, our Adults and Older People’s Social Services joined together with the local NHS Primary Care Trust to form something special – the Care Trust Plus. Since then, we’ve been leading the way in fully integrated care.
The first of its type in the country, the Care Trust Plus works with a combined budget of £270m to meet the health and social care needs of the 170,000 residents of the Immingham, Grimsby and Cleethorpes area. But it doesn’t end there. A redesign of the whole health and social care agenda, including the move of child health and public health services to the local authority, will allow more opportunity to tackle the wider issues of health and wellbeing, such as education, employment, housing and lifestyle.
There have been challenges along the way, and there will be more to meet, but our team has embraced this new way of working. They’ve also enjoyed more autonomy, more support, and more opportunities to enable truly individual care. We’ve heard a lot of positive things from them over the last year.
